NHS Sheffield are currently conducting an Audiology patient questionnaire, which will be used to gauge patient opinion about having future audiology appointments in community settings nearer to their homes.

Last summer, NHS Sheffield asked patients and the public for their views on health care in Sheffield in a major campaign called Big Health Conversation. The results showed that people want their services to be closer to home where possible.

NHS Sheffield is currently looking at the possibility of relocating audiology services into community locations and is seeking your views on improving access to the proposed community service. Your views will be used to inform the development of this service.  If you would like to complete the questionnaire, please follow the link below.
